The Medicare short stay review (SSR) process under the auspices of the Beneficiary and Family Centered Care Quality Improvement Organization (BFCCQIO) is entering its third year. This article is designed to provide a succinct summary of the current state of the process and address opportunities for improvement that we have identified since the process began. The BFCC-QIOs are committed to a fair and consistent adjudication process using the CMS algorithm for approval of short stay reviews. Inadequate documentation of the patient’s clinical condition, plan of care, and unusual events that inform the physician’s decision continues to be a challenge for the BFCC-QIO review process and a major reason for denials of Part A payment.
