Details have been added to describe Livanta’s services and technology platforms. In addition, website visitors will be able to easily locate key company information, including contract vehicles, and current and past performance.

About the BCMP
Managed by PRI, the BCMP offers certain Medicare beneficiaries personalized care management services. The BCMP works directly with beneficiaries who have a higher than average risk of readmission to the hospital. PRI care managers help Medicare beneficiaries, family members, and other caregivers manage care transitions and assist with healthcare navigation.
Livanta partners with PRI in 27 states and territories by determining whether patients who filed an appeal with Livanta’s case review BFCC-QIO meet criteria for referral to the BCMP.
Livanta’s clinical staff introduce the BCMP program over the phone by describing how the program works and its potential for positive benefits. For beneficiaries or families who are interested in the BCMP, their call is transferred immediately to PRI for follow-up. Once accepted into the BCMP, Medicare beneficiaries receive regular check-ins and telephone discussions.
